Multi-specialty clinic will adopt combined Phytel and Verisk Health technologies to drive successful participation in the Medicare Shared Savings Program and commercial risk-based contracts
SEATTLE, April 10, 2013 — The Polyclinic, one of the largest multi-specialty clinics in the Puget Sound region, has selected Phytel’s Population Health Management solution and Verisk Health’s Provider Intelligence solution to deliver a population health management platform that enables its more than 200 physicians to deliver higher-quality care at a lower cost.
The Polyclinic is a pioneer in the healthcare industry’s transition from a fee-for-service to a value-based payment model in which providers are paid according to the quality of care they deliver. The clinic has partnered both with commercial payers on such contracts and with the Centers for Medicare and Medicaid Services (CMS) as an early participant in the Medicare Shared Savings Program. The solutions from Phytel and Verisk Health will deliver a key platform that links financial and performance analytics with case management and population health tools within existing care delivery models.
