Fair Credit Reporting Act

Under the Fair Credit Reporting Act (FCRA), a person who is the subject of a consumer report may obtain a copy if the report has been a factor in an adverse action against the consumer in connection with employment or insurance, such as denial of employment or insurance cancellation.

A consumer who wants to dispute the accuracy or completeness of information returned in the report must contact iiX within sixty (60) days of being notified of the adverse action.

Consumers can request a copy of the report from iiX using one of the options below:

  • Download and print the Release of Consumer Report Form.
  • Complete the form.
  • Sign and return the form.
  • iiX will mail the report to the address you enter on the release form.

Option 2—By telephone or fax

  • Call iiX at 1-866-560-7015 and provide your name, mailing address, and telephone number (with area code). If you fax the request, include your fax number. We may contact you if we need clarification.
  • iiX will mail or fax you the Release of Consumer Report Form.
  • Complete the form.
  • Sign and return the form.
  • iiX will mail the report to the address you enter on the release form.

Adverse action based on Criminal History Reports

A consumer who wants to dispute information returned on a Criminal History Report must contact iiX within sixty (60) days of being notified of the adverse action.

  • Complete the form.
  • Sign and return the form.
  • iiX will research the source of the data for accuracy.
