Reports on Consumers - Adverse Action

Under the Fair Credit Reporting Act (“FCRA”), a consumer who is the subject of a consumer report may obtain a copy of it if the report has been a factor in an adverse action against the consumer in connection with employment or insurance, such as denying employment or canceling insurance. The consumer may obtain a copy of the report if it is requested within sixty (60) days of being notified of the adverse action. The consumer may dispute the accuracy or completeness of any information in the consumer report.

If a consumer has had an adverse action taken against him or her, s/he may contact iiX within the 60-day period to request a copy of the report. iiX's process provides these alternatives for obtaining the report:

Option 1 - Request via the web

  • The consumer downloads and prints the Release of Consumer Report form  .
  • The consumer completes the form by providing required information, including his or her driver's license number, and providing authorization for iiX to locate the report and to release it to the consumer.
  • Once the consumer signs and returns the release, the report is mailed to the address shown on the release form.

Option 2 - Request via telephone or fax

  • The consumer contacts iiX via a toll-free number, 866-560-7015, and provides his or her name, mailing address, and full telephone number (with area code). If the consumer uses a fax machine, s/he should include that number. Someone from iiX may contact the consumer if clarification is needed.
  • iiX will then mail or fax the Release of Consumer Report form   to the consumer. The consumer completes the form by providing required information, including his or her driver's license number, and providing authorization for iiX to locate the report and to release it to the consumer.
  • Once the consumer signs and returns the release, the report is mailed to the address shown on the release form.

Adverse Action Based on Criminal History Reports

If a consumer would like to dispute information returned on a Criminal History Report, s/he may contact iiX within the 60-day period to do so. iiX's process provides these alternatives for disputing the report:

Option 1 - Request via the web

  • The consumer downloads and prints the Criminal History Reinvestigation Form  .
  • The consumer completes the form by providing required information, and providing authorization for iiX to locate the report.
  • Once the consumer signs and returns the Reinvestigation Form, the accuracy of the information is then researched with the source of the data.

Option 2 - Request via telephone or fax

  • The consumer contacts iiX via a toll-free number, 866-560-7015, and provides his or her name, mailing address, and full telephone number (with area code). If the consumer uses a fax machine, s/he should include that number. Someone from iiX may contact the consumer if clarification is needed.
