Verisk will provide one free Loss History Report in any 12-month period.

To request a free A-PLUS™ Loss History Report, call the A-PLUS Consumer Report Request Line at 1-800-627-3487.

The A-PLUS Consumer Report Request Line is available Monday through Friday from 7 a.m. to 9 p.m., Eastern time. During nonbusiness hours and on weekends and holidays, you may leave a message with your name and telephone number. A representative will return your call on the next business day.

We’ll send reports by mail only. Verisk will mail Loss History Reports within 15 days of the request. If you want to order more than one report within a 12-month period, you can do so for a fee by calling the A-PLUS Consumer Inquiry Center at 1-800-709-8842, option 2. We’ll send you a Request for Disclosure Form, along with instructions on how to complete and return the form. You must complete and return the form to request additional reports.
